news item - Memantine and donepezil combination in severe Alzheimer's diseaseA placebo-controlled clinical trial in patients with moderate to severe Alzheimer's disease (AD) suggests that memantine can add some benefit to existing treatment with donepezil. It involved 404 patients already stabilised on donepezil, who were given memantine or placebo daily for 24 weeks. Assessment was using a number of standard outcome measures for dementia, and 322 (80%) of patients completed the trial. The results favoured memantine over placebo for all measures, and the authors conclude that adding memantine to stable donepezil treatment gave significantly better outcomes than placebo. [Editor's comment: although a 20% dropout rate may seem quite high, it is probably not unreasonable for this group of patients. The changes in outcomes are statistically significant, but it's not clear whether they represent clinically significant improvements in functioning.
