Vascular Surgery(2006) Conversion from tacrolimus to ciclosporin in the treatment of new-onset diabetes millitus after renal transplantation Living related renal transplantation (Tx) was carried out on a 33-year-old female with end-stage renal failure due to IgA nephropathy. Diabetes or impaired glucose tolerance was not overt prior to Tx in this patient. She was placed on a triple immunosuppressive regimen consisting of tacrolimus, mizoribine and steroid. Immediately after Tx the graft functioned well. Tacrolimus trough serum levels were therapeutic and non toxic, and concomitant steroid dosage was decreased; however her blood glucose level rose to an extremely high level and insulin and oral antidiabetics were required. On the 31th day her glucose metabolic disorders persisted. Furthermore, cytomegalo antigenemia occurred and continued in spite of ganciclovir and high-titer immunoglobrin therapy. When the tacrolimus dose was reduced to 5mg bid daily and predonisolone 5mg every other day, we decided to switch immunosuppression from tacrolimus to ciclosporin. After conversion to ciclosporin therapy, she showed improvement of cytomegalo antigenemia, and normalization of glucose metabolism. On the 11th month after Tx impaired glucose tolerance and cytomegalo antigenemia are not appearant. (author abst.
