At our 59 bed adolescent Residential Treatment center, where the length of stay is between 3 16 months (70% females; 30% males) ~20% of patients are receiving lithium carbonate for suicidal and self-harm behaviors. At blood levels of 0.3 1.3 mg/mL (median dose: 0.5 mg/mL), lithium carbonate has produced a sustained remission in suicidal thinking beginning ~48 hours after the first dose and continuing until discharge. Self-harm behavior, in the form of cutting, also decreased. Thus, what had been a frequent behavior at our facility (39% of our patients currently have a history of cutting or suicide attempts) diminished to occasional superficial scratches for some patients and cessation in others.
